St Louis Mo – Tulsa Ok flight

Tuesday, April 20, 2004 in Kalaupapa, HI
Aircraft: Cessna 208B, registration: N302PW
Injuries: 1 Serious, 4 Uninjured.

On April 20, 2004, about 1530 Hawaiian standard time, a Cessna 208 airplane, N302PW, encountered moderate turbulence during cruise flight, while transitioning along the northern shores of the island of Molokai, Hawaii. The flight was conducted under Title 14, CFR Part 135, as a scheduled domestic passenger flight, operated by Pacific Wings LLC, as Flight 2005. Of the five people aboard, the certificated commercial pilot, and three passengers were not injured. The remaining passenger sustained serious injuries. Visual meteorological conditions prevailed, and company flight following procedures were in effect at the time of the accident. The flight originated at the Kahului Airport, about 1511, and was en route to the Honolulu International Airport, Honolulu, Hawaii.

In a written statement provided to the National Transportation Safety Board (NTSB), the operator's chief pilot reported that after the flight departed from the Kahului Airport, it climbed to 2,500 feet msl for the flight between the islands of Maui and Molokai. Once the flight was within gliding distance of the northern shore of Molokai, the flight descended to 1,000 feet msl.

The coastline along the northern shore of Molokai is flanked by a 3,000-foot high cliff. As the flight progressed westbound along the coastline, the airplane encountered turbulence, and dropped sharply. During the turbulence, a passenger seated in the rearmost row of seats, and on the right side of the airplane, struck his head on an air vent located just above his seat, and sustained a scalp laceration. Immediately after the turbulence encounter, the pilot climbed the airplane and proceeded directly to the flight's destination airport of Honolulu, and landed without further incident. After arrival in Honolulu, the passenger was transported by ambulance to a local hospital.

During a telephone conversation with the NTSB investigator-in-charge (IIC) on April 23, the passenger who was injured in the turbulence reported that following the accident, he was experiencing numbness in his left arm, and that his head laceration required stitches.

The 3 cockpit crewmembers, 9 cabin crewmembers, and 240 passengers were not injured. Night visual meteorological conditions prevailed for the flight that departed Naples, Italy; destined for Norfolk Naval Station (NGU), Norfolk, Virginia. An instrument flight rules flight plan was filed for the public use non-scheduled passenger flight. 252 uninjured.

Flt 2297. One F/A seriously inj

On July 15, 2004, at 2304 central daylight time, a McDonnell Douglas DC-9-82, N585AA, operated by American Airlines Inc., as flight 1114, encountered turbulence during descent through 17,000 feet mean sea level about 1.6 nautical miles southwest of Sheridan, Illinois. Visual meteorological conditions prevailed at the time of the accident. The 14 CFR Part 121 passenger flight was operating on an instrument rules flight plan. One flight attendant was seriously injured. The flight originated from Albuquerque International Sunport Airport, Albuquerque, New Mexico, at 1916 mountain daylight time, en route to Chicago O'Hare International Airport Chicago (ORD), Illinois, where it landed without further incident.
On September 13, 2004, at 2003 Pacific daylight time, a Boeing 737-3H4 transport category airplane, N601WN, operated by Southwest Airlines as flight 1437, landed uneventfully after encountering wake turbulence while on approach to the Los Angeles International Airport (LAX), Los Angeles, California. Two flight crew, 2 cabin attendants, and 42 passengers were uninjured, while 1 flight attendant sustained serious injuries. The airplane was undamaged. Southwest Airlines was operating the airplane, registered to a trustee and leased to Southwest, under the provisions of 14 CFR Part 121 as a scheduled domestic passenger flight. The flight originated from Salt Lake City, Utah, and was destined for LAX. Visual meteorological conditions prevailed, and an instrument flight rules flight plan had been filed.

According to Southwest Airline's safety department, the flight crew reported encountering wake turbulence while on approach. A flight attendant was injured in the cabin area. Southwest learned of the extent of the flight attendant's injuries (broken ribs) 2 days following the event and then notified the National Transportation Safety Board Southwest Regional Office.
On September 29, 2004, about 1345 eastern daylight time, a Boeing 767-232, N109DL, registered to and operated by Delta Air Lines Inc., as a Title 14 CFR Part 121 scheduled international air carrier passenger flight, had an encounter with turbulence, which resulted in an injury, while over the Caribbean Sea. Visual meteorological conditions prevailed, and an instrument flight rules flight plan was filed. The airline transport-rated pilot, first officer, 4 flight attendants, and 138 passengers were not injured. One flight attendant was seriously injured, and the airplane was not damaged. The flight originated in Atlanta, Georgia, the same day, about 1029.

An official with Delta Airlines stated that the airplane had been descending, and was about 20 minutes from landing in Aruba, Netherlands Antilles, when it encountered turbulence. He further stated that the seat belt sign had been illuminated, but a flight attendant who was standing in the aft galley, was lifted from her feet, and she came to rest on the floor of the airplane, receiving a serious injury as a result of the brief turbulence encounter.

CHICAGO (AP) — An American Airlines flight from New York hit air turbulence Wednesday, injuring four flight attendants. The plane made an emergency landing in Chicago.

Flight 117 from JFK Airport was headed to Los Angeles when the 767 experienced turbulence as it was climbing at 30,000 feet, said American Airlines spokesman Tim Wagner.

None of the flight's 137 passengers were injured because the sign to fasten their seat belts was on, Wagner said.

The plane landed at Chicago's O'Hare International Airport as a precaution so the injured flight attendants, suffering bruises or strained necks, could receive medical care.

The passengers transferred to a new aircraft — a Boeing 757 — so the 767 they were on could be examined for possible damage, Wagner said.

Passengers described several seconds of scary shaking after the plane landed Wednesday night at Los Angeles International Airport about 3 1/2hours late.

"It just hit us all at once, and it just, you know, shocked me," passenger Nancy Roncone told KABC-TV. "I thought maybe the plane cracked in half."

Passenger John Kiloran described the turbulence as "5 or 10 seconds of just like crash, bang, boom, up and down. And immediately they knew something was wrong with the stewardesses. They called anyone who was a physician, they asked for a doctor to come help."

7 Nov Orlando crew injured in flight from Puerto Rico

Five crew members aboard an Orlando-bound flight from Puerto Rico were injured Sunday evening when the 737 aircraft hit turbulence, authorities said. ATA Airlines Flight 533 hit clear air turbulence en route from San Juan, Orlando International Airport spokeswoman Carolyn Fennell said.

The flight with 207 passengers on board landed at the Orlando airport about 6:40 p.m., Fennell said. The five injured flight attendants were taken to Florida Hospital East.

The injuries were not life-threatening, Fennell said. Many of the injured people were thrown around the airplane during the turbulence and suffered neck, back and leg problems. Officials from ATA Airlines could not be reached for comment.
